Information we collect
We collect only what we need to provide our service.
- Contact and account details you give us, such as your name, email address and the state where you are located
- Booking details, such as your appointment time and the number of dogs your request covers, and your dogs' names
- Health information you share during booking and your evaluation, and the clinician's notes and findings
- Payment information, which is processed by our payment processor; we do not store full card numbers
- Messages you send to support@usapsdletter.com
- Basic technical data, such as browser type, device information, pages visited and approximate location from your IP address, collected through cookies and similar tools
How we use information
- To schedule and carry out your evaluation and, if appropriate, issue your letter
- To process payment only when a letter is approved
- To confirm your state so that only a clinician who may lawfully evaluate you does so
- To send you your letter, receipts and service messages
- To respond to support, correction and verification requests
- To keep required clinical and business records, prevent fraud and comply with law
- To understand how our website is used and improve it
How we share information
We do not sell your personal information, and we do not share your health information for advertising. We share information only as follows:
- With our licensed clinicians, who use it to evaluate you and issue letters
- With service providers that help us operate, such as payment processing, video or phone scheduling, email delivery and secure hosting, under agreements that limit their use of the information
- When you ask us to, such as confirming to a housing provider that we issued your letter; we do not share health details with third parties without your authorization except as the law permits
- When required by law, such as a valid subpoena or court order, or to protect someone's safety
- In connection with a merger, acquisition or sale of assets, subject to this policy
Health information
Health information connected with your evaluation is also described in our Notice of Privacy Practices, which explains how it may be used and disclosed and the rights you have over it. If the two documents differ for health information, the Notice of Privacy Practices controls.
Cookies and analytics
We use cookies and similar technologies to keep the site working, remember preferences and measure site traffic. You can block or delete cookies in your browser settings; some features may not work properly if you do. We do not place health information from your evaluation in advertising or analytics tools.
Data retention
We keep clinical records for as long as required by the laws that apply to our clinicians, and other information for as long as needed to provide the service, keep business records, resolve disputes and meet legal obligations. When information is no longer needed, we delete or de-identify it.
Security
We use administrative, technical and physical safeguards designed to protect your information, including encrypted connections and access limited to people who need it. No method of transmission or storage is completely secure, so we cannot promise absolute security. Please do not send detailed medical information by email.
Your privacy rights
You may ask to access, correct or delete your personal information, subject to records we must keep by law, and you may opt out of non-essential emails at any time. Depending on your state, you may have additional privacy rights. To make a request, email support@usapsdletter.com from the address you booked with; we will verify your identity before acting.
Children
Our service is intended for adults. We do not knowingly collect personal information from children under 13 through this website. If you believe a child has given us information, contact us and we will delete it.
Policy changes
We may update this policy. If we make material changes, we will post the new version here with a new effective date.
